Cognitive Wellness is a private-pay service that focuses on everyday thinking, memory, and communication skills as they relate to daily life. The service is designed for older adults who want support with staying organized, following routines, and communicating effectively, and for families who want clearer insight into how their loved one is doing.

What Cognitive Wellness Can Include:

    • Setting up and practicing routines for medications, appointments, and daily tasks

    • Using checklists, calendars, and visual supports to make routines easier to follow

    • Applying memory, attention, and problem-solving strategies to real-life situations

    • Planning and organizing tasks such as errands, bill payment, or meal preparation

    • Supporting word-finding and clarity in conversation

    • Practicing strategies for asking for repetition or clarification

    • Planning and rehearsing conversations with family members, caregivers, or providers

    • Preparing information and questions for medical appointments

    • Organizing instructions and follow-up steps in a usable format

    • Providing education on communication and cognitive changes

    • Discussing ways to support routines and safety at home

    • Collaboratively developing a simple home support plan

What does this service consist of?

  • A caregiver, family member, or client reaches out with a brief description of current concerns and goals.

  • A short consult is scheduled to determine whether Cognitive Wellness is an appropriate service and to select the best format (hourly, series, or retainer).

  • Sessions are used to learn about daily routines, strengths, and challenges, and to begin introducing strategies and supports.

  • Strategies are refined over time based on what is most effective and practical in everyday life. Family members may be included for education and consultation as needed.
